To reserve a subscription for a Meat, Poultry, Country Breakfast or Egg CSA offline, please complete the following steps:

1. Print the CSA Subscription Form

2. Fill out the subscription form and enclose your payment in full. Make sure checks are made out to Keith Cannon.

3. Mail your form and payment to:

Evansville, IN 47712

Once the form and payment are received, you will receive confirmation that your spot in the CSA has been reserved.
