Subscription Form

To reserve a subscription for a Meat, Poultry, Country Breakfast or Egg CSA, please complete the following steps:

1. Print the CSA Subscription Form

2. Fill out the subscription form and enclose your payment in full or half as detailed on the form. Make sure checks are made out to Keith Cannon.

3. Mail your form and payment to:

Once the form and payment are recieved, you will recieve confirmation that your spot in the 2010 CSA has been reserved.
